What are GLP-1 weight loss medications?
GLP-1 medications are prescription drugs that act like a hormone your gut releases after eating. They signal fullness to the brain and slow how quickly food leaves the stomach, which helps many people eat less without feeling deprived. Tirzepatide also acts on a second hormone pathway, GIP.
They are typically prescribed for adults with obesity, or overweight with a weight-related health condition, and they are meant to be used together with a reduced-calorie diet and increased physical activity.
- Semaglutide
Weekly injection. Sold as Wegovy® for weight management. In the STEP 1 trial, people lost about 15% of body weight on average over 68 weeks.1
- Tirzepatide
Weekly injection acting on GLP-1 and GIP. Sold as Zepbound® for weight management. In SURMOUNT-1, average loss was 15–21% over 72 weeks, depending on dose.2
- Oral options
Some programs offer daily tablets. Ask your provider how oral and injectable options compare for you.
Trial results come from brand-name medications studied together with diet and exercise. Individual results vary, and compounded versions have not been studied in these trials.
Compounded vs. brand-name medication
Most online programs offer compounded semaglutide or tirzepatide. These are prepared by state-licensed compounding pharmacies based on a provider's prescription, and they usually cost less than brand-name drugs paid out of pocket.
Important: Compounded medications are not FDA-approved. The FDA does not review them for safety, effectiveness or quality before they are sold, and they are not the same as Wegovy®, Zepbound® or Ozempic®. Ask your provider which pharmacy prepares your medication.
How online programs work
- Answer a health questionnaire. Usually takes a few minutes and covers your weight, health history and current medications.
- A licensed provider reviews it. Some programs add a video or phone visit. The provider decides whether a prescription is appropriate.
- Medication ships to you. If prescribed, it's sent from a partner pharmacy, often with free shipping.
- Ongoing check-ins. Your provider adjusts the dose over time and monitors side effects.
Not everyone qualifies. If you are not prescribed, check each program's refund terms before you pay.
Side effects and who should not use GLP-1s
The most common side effects are nausea, diarrhea, constipation, vomiting and stomach discomfort, most often when starting or increasing the dose.1,2 Less common but serious risks include pancreatitis, gallbladder problems and low blood sugar in people taking certain diabetes medicines.
Semaglutide and tirzepatide carry a boxed warning about thyroid C-cell tumors. They should not be used by people with a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).3,4 Tell the provider about your full medical history, pregnancy plans and all medications.
How to choose a program
- Look past the first month. Many programs advertise an introductory price. Check what you'll pay from month two, and whether the price rises at higher doses.
- Check what's included. Provider visits, messaging, shipping and supplies may be bundled or billed separately.
- Know what you're getting. Confirm whether the medication is compounded or brand-name.
- Read the refund and cancellation terms, especially what happens if you are not prescribed.
Frequently asked questions
Do I need insurance?
No. The programs on this page are cash-pay. Some accept HSA/FSA cards. Brand-name medication may be partly covered by some insurance plans.
Do I need a prescription?
Yes. Every program requires a medical evaluation, and medication is only shipped if a licensed provider prescribes it.
How fast will I lose weight?
It varies. In clinical trials, weight loss built gradually over many months while the dose was increased step by step, alongside diet and exercise.
Are compounded GLP-1s the same as Wegovy® or Zepbound®?
No. Compounded medications are not FDA-approved and have not been reviewed by the FDA for safety, effectiveness or quality. They are prepared by compounding pharmacies based on a prescription.
What happens if I stop?
Appetite usually returns, and studies show many people regain weight after stopping. Talk to your provider about a plan before you stop.
How we put this page together
We compared programs using information each provider publishes on its own website: price for a typical starting plan and after any introductory period, medication options, what's included (provider visits, messaging, shipping), and refund and cancellation terms. Prices were last checked on November 2, 2026. We did not receive treatment from these programs, and this page is not medical advice.
Scores reflect our assessment of price transparency, what's included, medication options and terms. Placement and order on this page are also influenced by the compensation we receive.
